You may be covered by Medicaid if you have high medical bills, receive Supplemental Security Income (SSI) or if you meet certain income, resource, age, or disability requirements. |

IF YOU ARE an AMERICAN CITIZEN AND APPLYING FOR EMNERGENCY MEDICAID COVERAGE FOR ABORTION SERVIES ON THE SAME DAY YOU MUST BRING THE FOLLOWING DOCUMENTS:
1. A current government issued ID such as a passport, drivers license, OR school id
2. You ALSO NEED an original birth certificate
3. Social Security Card WE DO NOT ACCEPT COPIES
4. Proof of residence, which is mail in your name TO YOUR HOME not more than 60 days old.
5. Proof of income in the form of the last four weeks pay stubs, SSI letter, Unemployment letter, or a letter of support.
IF YOU ARE A NON-CITIZEN APPLYING FOR EMERGENCY MEDICAID FOR ABORTION YOU ARE NOT REQUIRED TO BRING A SOCIAL SECURITY CARD.
IF YOU ARE A CITIZEN AND APPLYING FOR MEDICAD GOVERAGE FOR A SAME DAY GYNECOLOGY VISIT YOU MUST ALSO BRING IN YOUR SOCAIL SECURITY CARD AS WELL AS ALL:
1. a current government issued Id--passport drivers licence or school ID
2. ORIGINAL birth certificate(we do not accept copies)
3. Proof of residence,(mail to your home not more then 60 days old)
